Food Allergies and Intolerances

Food allergies can manifest throughout life, from childhood to adulthood. Symptoms can vary greatly, from a mild rash around the mouth to a severe anaphylactic reaction. It is crucial to identify the triggering food to provide proper instructions to avoid the allergen as well as hidden or cross-reactive sources. Diagnosis is based on a detailed clinical history, skin and laboratory tests, and in some cases, provocation or tolerance tests. In certain cases, molecular diagnosis is used to obtain a more precise allergy profile, especially in cross-reactivity syndromes (such as pollen and vegetable allergies, dust mites and crustacean shellfish, among others) and in complex cases. Treatment focuses on avoiding the triggering food and related items, as well as emergency measures in case of a reaction. In recent years, desensitization has been implemented as a treatment at the Asthma and Allergy Clinic, making it a pioneer in private medicine. This involves gradually administering increasing doses of the allergenic food to induce oral tolerance. It’s important to note that not all adverse reactions to foods are caused by an IgE‑mediated allergy. Sometimes reactions are due to other immune mechanisms or non-allergic intolerances, such as lactose intolerance. Our extensive clinical experience in food reactions also makes us experts in food intolerances.

Allergic Reactions to Medications

Adverse drug reactions include a wide variety of responses, such as known side effects, true allergic reactions, non‑allergic immune reactions, intolerances, or idiosyncratic responses. Not all reactions to medications are allergies. In fact, many people are mistakenly labeled as “drug‑allergic” due to incomplete or non‑rigorous evaluations. A detailed medical history is essential to identify, classify, and assess the severity of a reaction. The study is complemented by allergy tests (using the relevant medications), lab analyses, and in selected cases, provocation or tolerance tests, which must always be performed in a hospital setting due to potential risks.

Severe Allergic Reactions

Systemic allergic reactions (anaphylaxis and anaphylactoid reactions). These reactions involve multiple organs or systems and, in their most extreme form (such as anaphylactic shock or laryngeal edema), can be life-threatening. Causes may include food or drug allergies, insect stings (hymenoptera), radiological contrast media, internal disorders, or a combination of factors. Sometimes identifying the cause is easy; other times it requires detailed detective work. Diagnosis includes a detailed clinical history, allergy testing, lab analysis, and other complementary tests. It’s also crucial to provide patients with anaphylaxis with instructions and training on using epinephrine and other emergency medications to control future episodes. Respiratory Allergic Diseases

Environmental respiratory allergies (asthma and rhinoconjunctivitis). Rhinoconjunctivitis and asthma are the most common conditions in allergology. Typical symptoms of rhinoconjunctivitis include nasal and eye itching, eye redness, sneezing, tearing, watery nasal discharge, and nasal congestion. Asthma usually manifests as episodes of breathing difficulty, chest tightness, dry cough, and wheezing. Another, less frequent condition is extrinsic allergic alveolitis, which presents with flu‑like symptoms (malaise, fever) and breathing difficulties hours after exposure to the triggering agent. A precise diagnosis is essential to develop a comprehensive treatment plan. With scientific advances, molecular diagnosis is increasingly important to define the patient’s allergy profile. In some cases, the allergen may be found in the patient’s workplace, requiring specific history and tests. Dermatitis and Skin Conditions

Atopic dermatitis is characterized by eczema on the skin, showing redness, itching, fine scaling, and sometimes fissures. This condition has a strong allergic genetic basis, and in many patients, environmental or food allergens play a clear role. For this reason, we recommend allergy testing for all patients with atopic dermatitis. Contact dermatitis presents similar symptoms but appears on skin areas exposed to the offending agent, caused by either irritant or allergic mechanisms. The only appropriate treatment is avoiding the trigger, so patch testing is key. Herpetiform dermatitis can resemble atopic dermatitis but is associated with celiac disease and must be distinguished from it. Urticaria is characterized by redness, hives, and itching in various parts of the body, while angioedema involves swelling of deeper tissues like lips, eyelids, hands, or throat. These manifestations can be caused by food or drug allergies, contact with irritants, physical factors (heat, cold, pressure), infections, autoimmune diseases, tumors, or genetic defects. In many cases, a detailed clinical history and complementary tests can identify the cause, though a significant percentage remain idiopathic (unknown cause). Other skin conditions can also have allergic origins, with many caused by drug allergies. Insect Sting Allergies

Stings and bites from insects can cause health problems, including allergic reactions. In most cases, the sting or bite causes localized inflammation—swelling, redness, and sometimes itching—due to the body’s immune response. Depending on the insect and the person’s skin sensitivity, these reactions can be mild or moderate and are usually treated with oral antihistamines and topical corticosteroid creams. Avoid home remedies like mud, as they can increase infection risk. In some cases, a person may develop sensitivity (specific allergy antibodies) to insect venom, triggering severe and generalized reactions: hives in other body areas, breathing difficulties, dizziness, or malaise. This is more common with hymenoptera (bees, wasps, hornets, bumblebees). In such cases, allergy testing is essential to confirm sensitivity and determine if it is a true allergy. This is important because: Future stings could cause more severe or even life-threatening reactions. Allergy vaccines (immunotherapy) are available to treat sensitivity to hymenoptera venom, with cure rates between 80% and 99%.
